Oakridge Teen Health Center
Hackley Community Care’s new Oakridge Teen Health Center is now open and we are seeing new
students. Our doors officially opened to students just before the 2015 Christmas Holiday break. The
grand opening celebration was held January 13, 2016 to dedicate the new facility.
The goal of being able to provide healthcare services to the Oakridge community has been made possible by all of our donors.
We would especially like to acknowledge our major donors: Michigan Department of Health and Human Services (MDHHS), the Osteopathic FDN, ALCOA FDN, Eagle Alloy, Inc., the Health Project of Mercy Health, the Gerber FDN, Community FDN for Muskegon County Youth Advisory Council, The Fazakerley Family and the VanKampen Boyer Molinari FDN.
We appreciate everyone that has contributed to this campaign.
Our capital campaign is still underway to equip our dental operatories. It is our goal to be able to provide dental services by the end of the year.

Info and Hours
Remember that we can coordinate care with your current family physician and we can be your primary medical home if you do not have one. Please call 733-6830 for an appointment.
Students have access to care and services during regular school days between the hours of 7:30 am to 4:30 pm (we are closed from 12:30 to 1:00 for lunch) on Monday, Tuesday, Wednesday and Thursday. Our Friday hours are from 7:30 am to 11:30 am (we are closed from 12:30 to 1:00 for lunch). We encourage parents to complete and sign a consent form for their child to be seen at the health center. Our current focus is on Oakridge students ages 5 to 21. Children outside of that age range are able to be seen after school hours. The Teen Health Center is located in the Middle School in Room 14. There is an exterior entrance near the parking lot for access after 3:00 PM.
The Oakridge Teen Health Center accepts most private insurances, Medicaid and offers a Sliding Fee Scale program to those that qualify. We do not charge co-pays for anyone between the ages of 10-21.
